By signing below, I state and affirm that I have consented to ministry from a Freedom Ministry team of two or more people. I consent for them to minister tome in the areas of inner healing and deliverance. I understand and acknowledge that all team members, whether they are licensed orlay who are involved in this ministry, are not licensed or trained as psychotherapists, mental health professionals, or professional counselors.
All guidance, counsel, and advice that I receive will be solely based on Scriptural principles and Christian Biblical standards as spelled out in the Holy Bible, the written Word of God. I understand and acknowledge that there are no guarantees made, nor can be made, about my healing and/or deliverance. Those who minister to me do so under the guidance and direction of the Holy Spirit, listening to the best of their ability, for His voice regarding my healing and deliverance. I understand and acknowledge that if I report a crime (i.e., sexual abuse to a child, elder abuse, or assault) to a Freedom Ministry minister and name a victim, Freedom Ministry is required by law to report it to the authorities. We will also notify the lead pastor of Bethel Church of Nevada County.
Freedom Ministry must also call the authorities if you tell us you are planning to commit suicide and do not convincingly change your plans before leaving your deliverance session. By signing below, I state that I have voluntarily sought this ministry for myself and that I hereby release Bethel Church of Nevada County, and all those associated with and all volunteers working with Freedom Ministry, from any and all claims of actual or implied liability that may arise now, or in the future, as a result of the ministry I receive.
Instructions and Acklowledgements:
1. Freedom Ministry prayer team members are neither psychological nor medical professionals. They do not charge for their time. This is a prayer ministry only. The process will take a minimum of eight weeks and may take ten weeks or more. Be prepared to set aside time for your one-hour weekly appointments and time for reading and prayer assignments. A stable internet connection and email are required.
2. Pray before you begin to fill out this questionnaire. Ask the Holy Spirit to help and guide you and bring key things to your mind. Please add any information concerning background or problems that may provide additional pertinent information. Be sure to note incidents that resulted in trauma, great disappointment, or any hurts that come to mind. When names are requested, it is only for prayer purposes - first names only, please.
3. Since forgiveness of those who sinned against us is our greatest weapon in deliverance, we ask that you be willing to pray and forgive such people when necessary. You must be willing to do this in order to achieve and maintain your freedom. Before you start your time of ministry with us, we ask you to purchase and read the book, “Forgiveness” by Rodney Hogue. As you read, write out 3-5 insights from every chapter indicating what stood out to you, what you have questions about, what would help you to forgive, and any other highlights the Lord shows you. These insights need to be handed in at, or prior to, your first session.
4. Prior to beginning ministry appointments, we ask that you purchase “Battlefield of the Mind” by Joyce Meyer. Please read this meaningful book either prior to, or during the weeks of our ministry sessions. For each chapter write out 3-5 insights and email them in each week at your appointed time.
5. Full disclosure and complete honesty are required during our sessions. If you are unwilling to be completely honest, please discuss your concerns with your ministry leader. Also, please pray and wait until you feel you are ready to start, or if you would like to cancel your appointment. All of us have areas that are hard to forgive. We ask that you come with a willingness to allow the Holy Spirit to help you to forgive. Your complete freedom will not be satisfactory and may even be hurtful in the long run if we are unable to deal with your circumstances in depth and with complete honesty. This will be your safe place to trust in the Lord.
6. There will be at least one other person present to pray and assist. Be prepared for this. Confidentiality will be maintained. Be assured that “we have heard it all” and nothing shocks or angers us. We are here to aid in the healing process.
7. To protect your privacy, with the exception of your signed consent form, your completed questionnaire will be destroyed at the completion of your prayer sessions. No copies are kept on file for any reason.
8. If you are physically able, and you sense the Lord leading you, please add fasting to your prayer before your appointment. If you have mature Christian friends who know you have an appointment, ask them to fast and pray if you feel comfortable doing this. It usually adds strength, guidance, and rapidity to the process.
9. As a rule, Freedom Ministry will take you from conception to the present, with the intent to bless every stage of your life, while bringing healing to the hurt and pain that has happened on your journey. This is accomplished by ministering, repenting, renouncing, forgiving, letting go, dealing with generational iniquities, and soul ties. This is done with the purpose of setting the captive free, opening the prison doors, and closing the doors the enemy used for entry. As we move through the areas of your life, we will always pray God’s blessings on each area. Be warned that after deliverance, if a person invites the problem back, the latter state of the person may be far worse than the original, and much effort, energy and time will have been wasted. You must agree to this. Please discuss any issues of forgiveness with your ministry leader.
10. Before we begin our sessions, you will be asked to pray the following prayer aloud with a sincere heart: “I confess Jesus Christ to be my personal Savior. I renounce any oppression from the evil one in my life because of iniquity, transgression and sin of my parents, ancestors, or myself. I humbly ask God for release and cleansing through the blood of Jesus Christ. I repent from every sinful attitude, action, or habit of mine which does not glorify Jesus Christ and ask for forgiveness, release, cleansing, and wholeness. I renounce the devil and all demonic influences, bondages, dominions, and infirmities in my life. I ask you, Lord, for the release and freedom promised by Jesus Christ so that He maybe Lord of my total being and be glorified in all I say and do. In His name I pray. Amen.”
11. Please return this questionnaire to Freedom Ministry. After the director has received it, your ministry team leader will set up your first appointment. If you are unable to keep your appointment or plan to cancel, please contact your ministry leader or the director as soon as possible.
12. I have read the above instructions and agree to comply fully.
