WAIVER
In consideration of being permitted to participate in the wushu seminars, I, the undersigned, on behalf of myself and my heirs, executors, and administrators, hereby assume my own risks and agree NOT to hold Shaolin Kung Fu Academy, US Shaolin Kungfu & Zen Federation, United Martial Arts Training Center and their staff, instructors, affiliates and subsidiaries liable or responsible for any bodily injuries and/or property damage arising from the participation and/or involvement in such activity and other related activities such as summer camps, trainings, practices, demonstrations, performances, and exhibitions, inside and outside of the school premises.
