    I understand that Wildly Curious provides creative, practical and outdoor learning experiences that may include activities such as art, craft, woodworking with age-appropriate tools, cooking, gardening, nature exploration, walking, games and other supervised learning experiences.

    I acknowledge that participation in these activities involves inherent risks, including the possibility of minor injury such as cuts, scrapes, bruises, splinters or falls.

    I understand that Wildly Curious is committed to providing a safe learning environment, follows its Health and Safety policies and procedures, undertakes appropriate risk management, and provides reasonable supervision. However, I acknowledge that accidents can still occur despite reasonable care being taken.

    I give permission for my child to participate in the activities offered as part of the Wildly Curious programme, unless I have notified Wildly Curious in writing of any restrictions.

    I confirm that:

    The health and medical information I have provided is complete and accurate;
    I will notify Wildly Curious of any changes to my child's health, medical needs or emergency contact details;
    My child will be encouraged to follow the instructions of staff, respect programme expectations and behave safely towards themselves, others and the environment.
    I understand that Wildly Curious cannot eliminate all risks associated with participation and cannot accept responsibility for injuries that result from a participant deliberately disregarding safety instructions, engaging in unsafe behaviour, or acting outside the reasonable supervision or control of staff.

    If my child becomes ill or is injured and I cannot be contacted, I authorise Wildly Curious to seek appropriate first aid and, where considered necessary, obtain medical treatment or emergency assistance on my child's behalf. I understand that I will be responsible for any associated medical costs not covered by ACC.

    By submitting this form, I confirm that I have read and understood this declaration and consent to my child's participation in Wildly Curious programmes.

  • Thank you for completing the registration form for your child once we have recieved your form you will be sent information regarding the workshop/s you have enrolled for and an invoice will be generated.

    Please compete a separate form per child.

    All workshops will be starting the week beginning 20th July 2026 and will be at our studio location at Morris and James, 48 Tongue Farm Road, Matakana. 

    NB: Please note that your child does need to be four before they can start the Thursday program and must be fully toilet trained.
