Home and Heart Pet Care, LLC Policy, Terms, and Conditions Agreement

Please read agreement and sign prior to first scheduled visit.

All services agreed upon will be provided by Home and Heart Pet Care, LLC (HHPC) in a caring, reliable, and trusting manner. In reference to the services agreed to and express condition thereof, the Client will willingly waive and relinquish any and all claims made against HHPC except those that involve gross negligence or willful misconduct on the part of HHPC.

The Client agrees that this signed contract will be valid for all future services with HHPC.

The Client agrees to reimburse and is fully accountable for any and all additional fees for HHPC to address emergency or veterinary care as well as any other unexpected supplies (including but not limited to: food, repairs, cleaning, transportation, supplies, medication). The Client is aware and understands that veterinary fees may be costly, especially if surgery or hospitalization is involved.

If the client requires the use of HHPC veterinary nursing services while away: The Client understands that HHPC is not a veterinarian and therefore will not hold HHPC to circumstances beyond their control. The Client understands that HHPC will only perform nursing care as instructed by a veterinarian and will not be responsible for decisions regarding the pets’ health made by the veterinarian and owner.

I, the Client, have read and fully understand the above terms and policies. The client has also read and fully understands HHPC contractual agreement. This agreement will be valid from the date below and will include all pets in the Client's household.
