      • 1. General Liability Release 

        By signing below, I agree that the following are not responsible for any injury, illness, accident, death, property damage, or loss of any kind related to this event, whether caused by negligence or otherwise:

        • The Business of Dance LLC
        • Menina Fortunato (personally and professionally)
        • All employees, staff, contractors, instructors, choreographers, photographers, videographers, assistants, volunteers, agents, representatives, venue partners, and anyone connected to this event

        I voluntarily assume all risks and agree that I will not sue any of the above individuals or entities for any reason, including but not limited to:

        • Injury
        • Illness
        • Death
        • Accidents
        • Negligence
        • Property damage
        • Any action or situation arising from participation in this event

        This release applies to all activities before, during, and after the event.


        2. Signing on Behalf of Entire Party (Group Release)
        By signing below, I confirm that:

        • I am authorized to sign this release on behalf of every person listed on this form, including spouse, partner, children, family members, or guests attending with me.
        • Each person listed is covered by all waivers, including the liability release, medical release, media release, and all terms in this agreement.
        • I accept full responsibility for sharing this information with my entire party and confirming that they understand the terms.

        This means one signature covers the entire group, and all protections apply equally to each person listed.


        3. Medical Release
        If there is a medical emergency, I authorize event staff to obtain medical help for me or anyone in my party. I understand that I am responsible for any medical expenses.


        4. Media Release 
        I understand that photos, videos, audio, interviews, and behind-the-scenes content will be captured at Business of Dance VIP DAY Masterclass with Tina Landon and Travis Payne.

        By signing below, I give Menina Entertainment and The Business of Dance full permission to use the image, voice, likeness, and performance of myself and everyone in my party in any media, including:

        • Social media
        • Websites
        • Podcasts
        • Advertising
        • Promotional content
        • Magazine features
        • TV shows and streaming platforms
        • Documentaries
        • Billboards
        • Commercials
        • Press
        • Educational materials
        • Trailers and highlight reels
        • Any future Business of Dance or Menina Entertainment projects

        This permission is:

        ✔ Worldwide
        ✔ Unlimited
        ✔ Perpetual 
        ✔ Royalty-free
        ✔ Irrevocable
        ✔ For any purpose connected to Business of Dance or Menina Entertainment

        I understand the footage will not be sold as standalone stock footage.

        I waive the right to approve or review the final materials.


        5. No Refunds
        All ticket sales for Business of Dance VIP DAY Masterclass with Tina Landon and Travis Payne are final.


        6. Governing Law
        This agreement follows the laws of California, where the event is held.
        Because Menina Entertainment is based in Nevada, some matters may reference Nevada law or jurisdiction when appropriate.
