Dino Foam LLC Liability Waiver & Release

Please read this waiver carefully and complete all required fields before participating in a Dino Foam event.

 

THIS IS A RELEASE OF LIABILITY. PLEASE READ CAREFULLY BEFORE SIGNING.

I understand that participating in a foam party involves inherent risks, including but not limited to slipping, falling, collisions with other participants, skin or eye irritation, and other injuries that may occur during participation.

I voluntarily assume all risks associated with participation in the Dino Foam event. I confirm that I, and any minor participant for whom I am signing, am physically able to participate and have no known medical condition that would make participation unsafe.

I understand and agree that children must be supervised by a responsible adult at all times. Participants must follow all safety instructions provided by Dino Foam and must not engage in dangerous, reckless, or inappropriate behavior.

To the fullest extent permitted by Oregon law, I release and hold harmless Dino Foam, its owners, employees, contractors, representatives, and agents from claims arising from the ordinary risks associated with participation in the foam party, except where such a release is prohibited by law.

I accept responsibility for any damage caused by me or any minor participant under my supervision to Dino Foam's equipment or property due to intentional misconduct, reckless behavior, or misuse.

I understand that Dino Foam reserves the right to stop participation or end an event when unsafe conditions or behavior create a risk to participants, staff, equipment, or property.

FOR PARTICIPANTS UNDER 18

I certify that I am the parent or legal guardian of the minor participant named above, or that I otherwise have legal authority to sign on the minor's behalf. I have read and understood this waiver, voluntarily assume the risks associated with the minor's participation, and consent to the minor participating in the Dino Foam event.
