Induction Ceremony & Shipping Information
If you plan to attend your Chapter’s Induction Ceremony, your Membership Certificate, recognition items, and any additional purchased items will be shipped to your Chapter Advisor for distribution at the ceremony.

If you are unable to attend the ceremony, your materials will be shipped directly to the mailing address provided on this form. A flat $25 shipping fee must be selected and paid at checkout to receive your Membership Certificate, recognition items, and any purchased items by mail.

Refund Policy
The Gamma Beta Phi appreciates your membership within the Society. If a student desires a refund, that request must be submitted to National Headquarters within 30 days of the original induction payment.
