  • MEDICAL RELEASE In case of emergency, when the parents or guardians cannot be reached, I grant permission to St. Mark Catholic Church to provide and/or obtain medical attention for my child(ren). I realize that the recommendations of the emergency medical service will determine to which hospital my child will be taken if it is an extreme emergency. I further authorize that medical personnel of the hospital to care for and/or treat my child. Also, I agree on behalf of myself, to hold harmless and defend St Mark and the Diocese of Baton Rouge from any claim arising from or in connection with any illness or injury. Please check one.*

  • Parent's Promise

    We would like to enroll our child in the First Communion program at St. Mark Catholic Church in preperation for the celebration of First Holy Communion and Reconciliation in the spring.

    As parents, we hope to do our best at home to share our Catholic faith with our children.  We intend to be apostles in our house by living the faith, giving witness to, and leading our children to meet Jesus in the Sacrament of First Holy Communion.  It is our desire that we instill a fervent faith and constant hunger for the sacraments in our children.

    We agree that our children will take part in the faith formation being offered through St. Mark's Parish School of Religion, to actively participate in the second-grade preparatory meetings amd retreats required to fulfill the obligation for reception of this sacrament.

    In addition, we promise we will open our own hearts and mind to the Holy Spirit and ask God to deepen our own faith and love for Him and His Church.

  • Confirmation Parent’s Promise 

    Congratulations! Your child has elected to be a part of the journey to the sacrament of Confirmation.  As a family, you have been on this journey of life and faith with your son/daughter for quite some time.  This point is an important part of the road you have been traveling together.  It is an opportunity to stop and assess, where you have been, where you are and where you are headed, both in your life of faith and the life of faith of your child.  Perhaps this Confirmation preparation period is the perfect time for you, the parent, to identify how what you do as a parent, influences, or helps to build your child into a healthy adult.  Your child’s development of life skills is a process of achieving wholeness and “holiness”.  As important as faith may be, it is not a normal topic of dinner conversation in most families.  Most often young people need guidance and structure to place faith at the forefront of family discussions.  Creating an atmosphere for the nurturing of faith opens wonderful possibilities for your child to be a healthy, whole, and holy human being.  Simply taking an opportunity to reflect, share and pray together helps young people and parents express that faith is important to them and how they can find God in ordinary experiences of everyday life.  Remember the home if where the faith is developed and lived out. 

    I promise I will open my own mind and heart to the Holy Spirit and ask God to deepen my own faith and love for Him and His Church.
