Change of availability form must be submitted to a manager at least four (4) days prior to a new schedule being made.
A change of availability form is not an absence request. To request time off, please use the absence request form.
A change of availability form is only to be submitted for change in your schedule, i.e. change in school schedule, if you play sports, other commitments outside of work.
