I voluntarily consent to receive a spray tanning service from AP Gleaux Co.
I understand that spray tanning is a cosmetic service designed to temporarily darken the skin and does not provide protection from the sun or UV rays.
I acknowledge that results vary depending on my skin type, preparation, aftercare, medications, and lifestyle.
Medical Disclosure
I certify that I have informed my spray tan technician of any relevant medical conditions, allergies, skin sensitivities, medications, pregnancy, or other concerns that could affect my spray tan.
I understand that AP Gleaux Co. does not provide medical advice and that I am responsible for consulting my physician if I have concerns regarding this service.
Risks
I understand that although adverse reactions are uncommon, possible side effects may include:
Skin irritation
Allergic reaction
Uneven fading
Streaking
Color variation
Transfer of cosmetic bronzer to clothing or bedding
Dry skin
I understand these outcomes are not necessarily the result of technician error.
Client Responsibilities
I understand that achieving the best results depends on following all pre-care and post-care instructions provided by AP Gleaux Co.
Failure to properly prepare for or care for my spray tan may affect my results.
Limitation of Liability
I release and hold harmless AP Gleaux Co., its owner, employees, and representatives from any liability, claims, damages, injuries, allergic reactions, dissatisfaction with cosmetic results, or losses arising from my spray tanning service, except where prohibited by law or caused by gross negligence or intentional misconduct.
I understand that no guarantees have been made regarding the exact color, longevity, or appearance of my spray tan.
Photography Release (Optional)
☐ I give permission for AP Gleaux Co. to take before-and-after photographs of my spray tan.
☐ I authorize AP Gleaux Co. to use these photographs for advertising, social media, and marketing purposes.
☐ I do not consent to photographs.
