Waiver Agreement:
I understand that yoga involves physical movement and carries a risk of injury, both minor and serious (including sprains, strains, and in severe cases, permanent disability or death). I voluntarily choose to participate in yoga classes, workshops, private sessions, and other activites offered by Cassandra Chisholm, Yoga at Twilight, or any of its other teachers. I assume full responsibility for any and all risks, injuries, or damages, known or unknown, which I might incur as a result of my participation in all activities.
In consideration of being permitted to participate in yoga classes, private sessions, or other activities I, for myself, my heirs, and my assigns, hereby release, waive, and hold harmless Yoga at Twilight, its owners, instructors, employees, and independent contractors from any and all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses, of any nature, occurring during or resulting from my participation in any Studio activities.
I understand that it is my responsibility to participate only to the extent that I can do so safely and to consult an appropriate healthcare provider if I have any medical, physical, or mental health condition that could affect my safe particaption. I agree to practice within my own abilities and limitations and discontinue and inform the instructor should I experience pain, diziness, light-headness, shortness of breath beyond normal exertion, or any other concerning symptoms during any activity. I agree to inform my instructor of my pre-existing conditions that may impaact my participation, including pregnancy. I accept ongoing participation by these standards before and throughout each class or session.
In the event of an emergency, I authorize the Studio and its staff to seek emergency care on my behalf. I understand that I am financially responsible for any costs associated with emergency medical care, transportation, or hospitalization that I incur.
I have read this Release and Waiver of Liability and fully understand its terms. I acknowledge that I am signing this agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
