IDEAL PLASTIC SURGERY

THIS AGREEMENT is entered into by and between the undersigned individual (hereinafter referred to as "Member") and Ideal Plastic Surgery (hereinafter referred to as "Company").

WHEREAS, Member desires to participate in the Company's Membership Program to receive discounted pricing and accumulate treatment credits; and WHEREAS, Company agrees to provide said services subject to the terms and conditions set forth below;

NOW, THEREFORE, in consideration of the mutual covenants contained herein, the parties agree as follows:

1. MEMBERSHIP STRUCTURE & TIERS The Member hereby selects the following Membership Tier (check one):

• Benefit: 10% discount on eligible medspa services/products.
• Surgical Benefit: 5% discount on surgery (Available after 12 months of active membership).

Silver Membership ($99/month)

• Benefit: 10% discount on eligible medspa services/products.
• Surgical Benefit: 5% discount on surgery (Available after 12 monthsof active membership).

Gold Membership ($199/month)

• Benefit: 15% discount on eligible medspa services/products.
• Surgical Benefit: 10% discount on surgery (Available after 12 months of active membership).

Platinum Membership ($249/month)

• Benefit: 20% discount on eligible medspa services/products.
• Surgical Benefit: 15% discount on surgery (Available after 12 months of active membership).

2. THE "BANK" SYSTEM (MEMBERSHIP CREDITS)2.1. Accumulation.
Monthly Membership Dues paid by the Member are converted 1:1 into
"Membership Credits" on the Member's account. 2.2. Redemption.
Accumulated credits may be applied toward the purchase of eligible
MedSpa services and products. 2.3. No Cash Value. Membership Credits
are not a bank account, have no cash value, cannot be redeemed for
cash, and cannot be withdrawn. They exist solely as a prepayment for
future services.

3. DISCOUNTS & EXCLUSIONS3.1. Standard Pricing. Membership
discounts apply exclusively to regularly priced services and products.
3.2. Exclusions. Discounts cannot be combined with other promotions,
limited-time specials, package pricing, or offers unless explicitly
approved in writing by Company management. 3.3. Timing. Discounts
are applied at the point of sale when services are rendered or products
are purchased. 3.4. Surgical Benefit Conditions. The Surgical Discounts
outlined in Section 1 (Silver 5%, Gold 10%, Platinum 15%) apply strictly to
surgical procedures. These discounts become available only after the
Member has completed twelve (12) consecutive months of active
membership. This 12-month waiting period is mandatory, and no
exceptions will be made.

4. ELIGIBLE SERVICES & MEDICAL DISCRETION 4.1. Scope. Credits and
discounts may be applied to services including, but not limited to:
Facials, massages, Hydrafacial, microneedling (w/wo PRP), chemical
peels, laser treatments, scar revision, botulinum toxin, dermal fillers,
and medically supervised weight-loss programs (subject to eligibility).
4.2. Medical Necessity & Suitability. Enrollment in this Membership
does not guarantee candidacy for any specific medical treatment. All
medical services require a consultation and are subject to the sole
clinical discretion of the Provider. 4.3. Refusal of Service. The Company
reserves the right to refuse treatment if it is deemed medically unsafe
or unsuitable for the Member. Such refusal does not constitute a
breach of this Agreement.

5. BILLING, AUTHORIZATION, AND PAYMENTS5.1. Recurring Billing. The Member authorizes the Company to automatically charge the credit/debit card on file for the Monthly Membership Fee associated with the selected tier. 5.2. Payment Updates. Member agrees to maintain valid payment information on file. It is the Member's responsibility to update the Company regarding any changes to the card number or expiration date. 5.3. Default. In the event of a declined payment, Membership benefits (including discounts and access to accumulated credits) will be immediately suspended until the account is brought current.

6. NO REFUND POLICY & WAIVER OF DISPUTE RIGHTS 6.1. Non- Refundable Dues. All Membership Fees paid are final and non- refundable. Fees are charged in exchange for immediate access to the discount structure and membership availability, regardless of whether the Member utilizes services during that month. 6.2. Chargeback Waiver. By signing this Agreement, the Member acknowledges that all charges are authorized. Initiating a credit card dispute or "chargeback" for valid Membership Fees constitutes a material breach of this Agreement and may result in immediate termination of the Membership and forfeiture of benefits.

7. TERM AND CANCELLATION7.1. Minimum Commitment. This Agreement requires a mandatory minimum commitment of twelve (12) consecutive months ("Initial Term"). 7.2. Early Termination. Cancellation requests prior to the completion of the Initial Term will not be accepted. Fees charged during the Initial Term are non- refundable. 7.3. Cancellation Procedure. After the Initial Term, Member may cancel the Membership by submitting a written notice to the Company. Cancellation will become effective following the conclusion of the current billing cycle. 7.4. Effect of Termination.

• Discounts: Upon cancellation, all Membership Discounts immediately cease.

• Unused Credits: Any remaining accumulated credits may still be redeemed for services or products; however, such redemptions will be charged at full, non-member pricing.
• 8. GENERAL PROVISIONS8.1. Non-Transferability. Membership benefits, including discounts and accumulated credits, are non-transferable and may not be shared, gifted, sold, or assigned to any other individual. 8.2. Membership Freeze. The Company may, at its sole discretion, grant one (1) courtesy membership pause per calendar year for documented medical reasons. Requests must be in writing. 8.3. Amendments. The Company reserves the right to modify pricing, benefits, or the terms of this Policy at any time.

By signing below, I acknowledge that I have read and understood this Ideal Glow Club Membership Agreement. I authorize the recurring monthly charge for the tier selected above and agree to the No Refund and Minimum Commitment policies.

• The "Bank": Your monthly fee sits in your account as credit to use later.
• The Discount: You get 10%, 15%, or 20% off (depending on tier) while your membership is active.
• The Commitment: You must stay enrolled for at least 12 months.
• No Refunds: Monthly fees cannot be refunded.
• Cancellation: If you cancel, you lose the discount, but you can still use your saved money (credits) to pay for services at the full standard price.
• Just for You: Credits cannot be shared with friends or family.
