This form is to be completed by the applicant's Director of Graduate Programs. Only one DGP authorization is necessary for each applicant, regardless if the applicant is considering to one or both awards.

Certify that the following is true. Fill in the blanks in the paragraph as well as the information below to complete the DGP Authorization for the applicant of the GSA Award for Conferences and/or GSA Travel Assistance Awards.

Please note that if the volume of applicants is high, the number of students you are able to authorize may be restricted to 5% of the number of graduate students in your program.
