Plant Powered Metro New York will present the next 21-Day Plant Powered Jumpstart beginning October 8.
A generous donor is providing a $150 participation incentive for eligible individuals who have been diagnosed with chronic kidney disease, stages 3, 4, or 5, including kidney failure.
The Jumpstart is free for all participants. This application is only for individuals with chronic kidney disease who would like to be considered for the additional completion incentive.
Applicants must live in a time zone that allows them to participate in the Jumpstart’s live virtual programming, including weekly educational sessions and mentorship meetings.
To receive the $150 incentive, selected participants must:
- Complete their Jumpstart registration
- Attend all live educational sessions
- Attend at least 2 of the 3 mentorship meetings
- Create a profile and participate in the virtual forum accompanying the live program.
- Complete the pre-program and post-program surveys
Attendance will be recorded at all sessions. Participants who meet the completion requirements will receive the $150 incentive after the program concludes.
This form must be submitted no later than 5:00 PM ET on Friday, October 2.
At any time before the registration deadline on Tuesday, October 6, you may complete the Jumpstart registration form, whether or not you've been notified about receiving an incentive.
