  • Is IOP Right for My Teen?

Choosing the right level of mental health care for your teen can feel overwhelming. This brief assessment is designed to help you better understand which level of care may be appropriate based on your responses. It is intended for informational purposes only and is not a substitute for a professional evaluation. A comprehensive clinical assessment is necessary to determine the most appropriate treatment recommendations. If, after speaking with our clinicians, another level of care is indicated, we will help guide you to the appropriate resources and remain available for continued care when appropriate. If your teen is in immediate danger of harming themselves or others, call 911 or go to the nearest emergency department or behavioral health crisis center immediately. If you are in the Triangle, local options include the Hope Center in Fuquay-Varina. If you are in the Charlotte region, Atrium Behavioral Health in Cornelius is another option. You can also call or text 988 to reach the Suicide & Crisis Lifeline.
  • Has your teen attempted suicide or engaged in a suicide attempt within the past 30 days?
  • Is your teen currently expressing thoughts of suicide?
  • Are you able to keep your teen safe at home until they can be evaluated by a mental health professional?
  • During the past two weeks, how much have your teen's mental health symptoms interfered with daily life?
  • Has your teen been able to attend school or complete their usual daily responsibilities during the past two weeks?
  • Has your teen participated in outpatient mental health treatment within the past 3 months?
  • Has your teen continued to experience significant mental health symptoms despite participating in an Intensive Outpatient Program (IOP) or Partial Hospitalization Program (PHP) within the past 3 months?
  • Is there a parent, guardian, or other responsible adult who is able to provide consistent supervision and participate in treatment?
  • Which of the following concerns is your teen experiencing? (Select all that apply.)*
  • Immediate Emergency Evaluation Recommended

Based on your responses, your teen may require immediate evaluation in an emergency department or psychiatric hospital. This recommendation is typically made when there are concerns about immediate safety, such as a recent suicide attempt, active suicidal thoughts with a plan or intent, inability to remain safe at home, or other symptoms that require urgent intervention.

If your teen is in immediate danger of harming themselves or someone else, call 911 or go to the nearest emergency department immediately. You can also call or text 988 to reach the Suicide & Crisis Lifeline.

Once your teen has been medically and psychiatrically stabilized, additional services such as outpatient therapy, Intensive Outpatient (IOP), or Partial Hospitalization (PHP) may be recommended as part of their ongoing care.

  • Residential Treatment May Be the Most Appropriate Level of Care

Based on your responses, your teen may benefit from a highly structured residential treatment program. Residential treatment provides 24-hour therapeutic support for adolescents whose symptoms significantly interfere with daily functioning or who cannot be safely managed in a less restrictive setting.

Residential treatment may be appropriate when outpatient therapy, IOP, or PHP have not been sufficient, when symptoms continue to worsen, or when consistent supervision and intensive therapeutic support are needed.

A comprehensive clinical evaluation is necessary to determine whether residential treatment is the most appropriate recommendation.

If you would like assistance understanding treatment options or identifying appropriate resources, our team is available to help.

  • A Partial Hospitalization Program (PHP) May Be Appropriate

Based on your responses, your teen may benefit from a Partial Hospitalization Program (PHP). PHP provides intensive treatment during the day while allowing teens to return home with family in the evenings.

PHP is often appropriate for adolescents experiencing significant emotional or behavioral difficulties that require more support than outpatient therapy or Intensive Outpatient (IOP), but who can safely remain at home outside of treatment hours.

Treatment typically includes individual therapy, group therapy, family involvement, psychiatric care, and medication management when appropriate.

A comprehensive clinical evaluation is recommended to determine whether PHP is the most appropriate level of care.

Please remember: This assessment is a screening tool and is not a diagnosis or substitute for a comprehensive evaluation by a licensed mental health professional.

  • An Intensive Outpatient Program (IOP) May Be the Right Fit
Based on your responses, your teen may benefit from an Intensive Outpatient

Program (IOP). IOP provides more support than weekly outpatient therapy while allowing teens to continue living at home and, in many cases, attending school.

IOP is often recommended for adolescents experiencing moderate to severe anxiety, depression, emotional dysregulation, school refusal, self-harm behaviors, or other mental health concerns that have not improved with traditional outpatient treatment alone.

At Krippa Family Psychological & Wellness Services, our adolescent DBT-based IOP combines evidence-based treatment with individual therapy, family involvement, experiential activities, and psychiatric support when needed.

If you would like to learn more about whether our IOP may be appropriate for your teen, please contact our admissions team. We are happy to answer your questions and discuss treatment options.

Please remember: This assessment is a screening tool and is not a diagnosis or substitute for a comprehensive evaluation by a licensed mental health professional.

  • Outpatient therapy might be appropriate:

Based on your responses, outpatient mental health services, such as individual therapy, may be an appropriate starting point for your teen. Individual therapy can help address concerns such as anxiety, depression, stress, behavioral challenges, family conflict, or school-related difficulties while allowing your teen to continue their normal daily routines.

Every adolescent is unique, and an individualized assessment by a licensed mental health professional is the best way to determine the most appropriate level of care. Depending on your teen's needs, recommendations may include individual therapy, family therapy, medication management, psychological evaluation, or another outpatient service.

This questionnaire is intended for educational purposes only and is not a diagnostic tool or a substitute for a comprehensive clinical assessment. If your teen is experiencing thoughts of suicide, is unable to remain safe, or you believe there is an immediate emergency, call 911 or go to the nearest emergency department.
