TAMPA SPORTS AUTHORITY BOARD & COMMITTEE MEETING REQUEST FOR PUBLIC COMMENT

Thank you for your interest in speaking at an upcoming Tampa Sports Authority Board or Committee meeting. Please complete the form below to request an opportunity to provide public comment either in person or virtually.  

Requests to speak, whether in-person or virtually, must be submitted no later than 15-minutes before the scheduled meeting time.  If you plan to speak in-person, you may use this form to pre-register or you may regsiter prior to the meeting start time when you arrive in the main lobby. 

Speakers will be called upon by the Board Chair by name in the order requests are received, on a first-come, first-served basis. Each speaker may speak for up to 3 minutes.

SPEAKING IN-PERSON

• Boardroom seating for visitors attending public comment is limited. If the Boardroom reaches capacity, overflow viewing will be made available.
• In-person public comment takes place in the Tampa Sports Authority Board Room, located at the Tampa Sports Authority Office inside Raymond James Stadium, 4201 N. Dale Mabry Hwy., Tampa, FL 33607.

 SPEAKING VIRTUALLY

• If you request to speak virtually, meeting participation instructions will be e-mailed to the address you provide on this form. Callers will be muted when they join and will be unmuted in submission order after the Chair recognizes them by name.
• For virtual participation, you must call in using the same phone number you provide on this form so staff can identify you and match your name to your phone number.
• If you do not receive the participation instructions by e-mail from: meetings@tampasportsauthority.com, please check your junk or spam folder.

**IMPORTANT**

If you are participating virtually, you MUST provide a valid phone number and call into the conference with the same number you provide on this form. This is so that staff can identify you and associate your name with your phone number.  If a phone number is not provided on this form, or the number you provide is not the same number you use to call in to the meeting, we will not be able to properly identify you and you may not be able to participate.

**IMPORTANT**

If you do not have an e-mail you must write down the confirmation number, print out, or take a screenshot of the confirmation screen after you press the submit button to have confirmation that you signed up to speak at the meeting.
