    This form is for rising 1st-6th graders. No audition is required to register for these grades. All 7th-12th graders have a separate form.

    1. Registration for the fall will close on September 28th.
    2. This form should be completed by a parent/guardian with the student nearby to assist with student-specific questions.
    3. You will be able to save your answers without submitting by clicking "Save" at the bottom of this form. You will receive an email from Jotform to continue your form when you are ready.
    4. If you are registering for more than 1 student, please fill out a separate form for each student.

  • Season Information

  • All SLYC ensembles will meet on Monday nights at Willard Middle School. Students will rehearse in individual choirs as follows:  Spark (1st-2nd grade) from 5:00-5:45, Ignite (3rd-4th grade) from 5:00-5:45, and Enchant (5th-6th grade) from 6:00-7:00.

    • Tidings of Joy concert:  Saturday, December 12th, 4:00pm

  • NEW & IMPORTANT UPDATE PLEASE READ:

    This year, we are implementing a new platform called Chorus Connection, which is a one-stop-shop for all things SLYC! Once registered, the Primary Parent/Guardian listed below will receive an invite. Please indicate if you would like to request a second parent/guardian account. Keep in mind, the price of the platform is determined per user, so we prefer to limit family access to one adult. It is at your discretion if you would like to share your login information with additional adults. Any student that is under 13 years old will only have a parent profile. Once a student turns 13, they will be eligible for their own profile.

    Accepted payments include Zelle (no fee) or debit/credit card (3% fee).

  • As part of Chorus Connection, payments will be tracked in the portal. You can either send Zelle for no fee (full payments only), or once you submit this form, we will assign your fee to your Chorus Connection account.

    Full Season October - April Payment Options:
    1. Pay in full: $500 for the year
    2. Pay in installments (not available for Zelle payment): 3 monthly payments of $200 for a total of $600

    Single Semester Payment Option:
    Each semester is $275

    REFUND POLICY

    Before August 15:  100% tuition refund, less a $25 administrative fee
    August 16 - September 28:  50% tuition refund, less a $25 administrative fee
    September 29+: No tuition refunds

    Exceptions
    Refunds or credits may be considered at the sole discretion of SLYC for extraordinary circumstances such as serious illness or medical emergency, family relocation, or other unforeseen circumstances approved by the Executive Director. Any approved exception may be issued as a prorated refund, a credit toward a future SLYC program, or another accommodation determined by SLYC.

    Installment Plans
    Families choosing an installment payment plan are committing to the full selected tuition. Withdrawal from the program does not cancel the remaining financial obligation except as outlined in this refund policy.
