By signing this Cancellation Request Form, I acknowledge that I am requesting to cancel my membership with 949 Powerlifting. I understand that submitting this request does not automatically terminate my membership and that all terms of my Membership Agreement remain in effect until my cancellation request has been processed in accordance with the agreement. I understand that cancellation of my membership does not release me from any outstanding balances, unpaid membership dues, fees, declined payment charges, or other financial obligations incurred prior to the effective cancellation date. I understand that my access to the facility will terminate on the effective cancellation date. Any attempt to access the facility after cancellation may be denied. I understand that membership dues are non-refundable except where required by applicable law or specifically stated in my Membership Agreement.
