Bags of Grace Volunteer Sign-Up & Waiver

Bags of Grace Volunteer Liability Release and Waiver of Claims. 501(c)(3) Nonprofit Organization. Purpose of Volunteer Activities: Bags of Grace is a registered 501(c)(3) nonprofit organization that provides bags of nonperishable food and hygiene items to persons experiencing homelessness. Volunteers may participate in the preparation, transportation, and distribution of these bags at various public and private locations. Terms of Agreement: 1. Assumption of Risk - I understand that participation in Bags of Grace activities may involve certain risks, including but not limited to physical injury, exposure to unpredictable environments, and interaction with individuals in vulnerable circumstances. I voluntarily assume all risks associated with my participation. 2. Release of Liability - I hereby release, waive, and discharge Bags of Grace, its officers, directors, board members, employees, agents, and volunteers from any and all liability, claims, demands, or causes of action that may arise from any injury, illness, loss, or damage incurred while participating in Bags of Grace activities, whether caused by negligence or otherwise. 3. Indemnification - I agree to indemnify and hold harmless Bags of Grace and its representatives from any claims, liabilities, damages, or expenses (including attorney's fees) arising from my actions or omissions during volunteer activities. 4. Medical Treatment - I authorize Bags of Grace to seek emergency medical treatment on my behalf if necessary. I understand that I am responsible for any medical costs incurred. 5. Photographic Release - I grant permission for Bags of Grace to use photographs or video footage taken during volunteer activities for promotional or educational purposes. 6. Acknowledgment of Understanding - I have read this release and waiver of liability in its entirety. I fully understand its terms and sign it freely and voluntarily.

Parental Consent and Supervision Agreement (For Volunteers Under Age 18). As the parent or legal guardian of the minor named below, I give permission for my child to participate in volunteer activities with Bags of Grace. I understand the nature of these activities and the potential risks involved. I acknowledge that: I am responsible for the supervision of my child during all Bags of Grace activities. I will ensure that my child is accompanied by a responsible adult at all times. I have reviewed the terms of the Volunteer Liability Release and Waiver of Claims and agree to its provisions on behalf of my child. I release Bags of Grace, its officers, directors, board members, employees, agents, and volunteers from any liability for injury, illness, or damages that may arise from my child's participation.
