Consultation Policy & Scheduling Disclaimer. If you select a Zoom (audio) consultation, a Zoom link will be sent to the email address provided on this form prior to your scheduled appointment time. Please check your inbox (and spam folder) before your appointment. If you are unable to attend your scheduled consultation, please notify us by email as soon as possible. The $99 consultation fee is non-refundable: if the appointment is canceled less than 5 days before the scheduled consultation date, or you do not attend the scheduled appointment ("no-show"), if we are unable to reach you at the phone number provided during your scheduled appointment time, or you do not join the Zoom meeting or answer the scheduled phone call within 10 minutes of the appointment start time.
