By signing below, I confirm that I am authorized to approve this project for the Client. I authorize Plexamedia to create, edit, submit, publish, and distribute the approved AI-assisted commercial video for the project listed above, subject to the selections and rules below.

- If a real person's likeness or voice is used, I confirm that the person has given permission, or I am that person and I give permission by signing this form.
- Plexamedia will not intentionally reuse a custom avatar, likeness, or voice model for another client or unrelated campaign without separate written approval.
- The video may not create a fake customer, fake patient, fake testimonial, or misleading endorsement.
- Client confirms that all business facts, offers, claims, testimonials, and before/after statements supplied by Client are accurate and approved.

If left blank, Plexamedia may use the most accurate disclosure from its AI Commercial Compliance Guide.

The final video is not released until Client replies in writing or clicks approval using the exact words: APPROVED FOR RELEASE. Email approval counts as written consent. If the Client, authorized approver, or likeness/voice signer replies to Plexamedia by email with APPROVED FOR RELEASE or I APPROVE THIS CONSENT FORM, that email will count the same as signing this form, as long as it clearly identifies the project or video. After approval, Plexamedia may release the approved version only through the authorized channels above. Material changes require new approval.

Note: If the signer is not the person whose likeness or voice is used, Client must keep that person's written permission on file or have that person sign this same form.
