MG 3:66 Presntations Liabilty, Consent & Media Release Waiver:
By allowing my child to participate with the above‑named team, I acknowledge and agree to the following:
I give permission for my child to participate in MGPresentations366 events.
I understand that participation in dance and majorette competitions involves physical activity and inherent risks.
I certify that my child is physically able to participate.
I authorize emergency medical treatment for my child if I cannot be reached.
I agree that MGPresentations366, its staff, volunteers, judges, sponsors, venue partners, and affiliates shall not be held liable for injuries, illness, accidents, lost or stolen property, or damages arising from participation, except where prohibited by law.
Photo & Video Release
I grant MGPresentations366 permission to photograph, record, and use my child’s image, likeness, voice, and performances for promotional materials, advertising, social media, websites, livestreams, broadcasts, printed publications, and future marketing without compensation. I understand that all images and recordings become the property of MGPresentations366.
Accuracy Certification
I, Parent/Guardian Name, acknowledge that the information provided in this roster form is accurate and up‑to‑date. I understand the importance of submitting correct information, especially regarding performers’ ages. I further understand that any inaccuracies may result in consequences for the team or individuals involved, including forfeits or suspensions.
