    Our goal is to keep more dogs in loving homes, off the streets, and out of shelters through spay and neuter programs and responsible adoption practices. 

    This application helps us learn about you, your household/family, and your lifestyle so we can make the best possible match. 

    Please answer each question honestly and completely. There are no "right" or "wrong" answers. Your responses help us make informed placement decisions that are in the best interest of both you and the dog. 

    Your privacy is important to us. All information submitted through this application will be kept confidential and used solely by The Canine Condition Foundation for the purpose of evaluating your adoption application. Your information will never be sold or shared outside of the adoption process without your permission unless required by law.

    Completing this application does not guarantee approval or obligate you to adopt a dog. The Canine Condition Foundation carefully reviews every application and reserves the right to approve or deny any application based on what we believe is in the best interest of each dog.

  • Even the most careful pet owners can experience the unexpected. A registered microchip is one of the safest and most effective ways to help a lost pet find its way home. All of the dogs we adopt out are microchipped and will be registered to their adopters.

  • Thank you for taking the time to complete our dog adoption application. We know adopting a dog is a significant decision, and we sincerely appreciate your interest in providing a loving, lifelong home for one of our dogs. 

    If your application is selected to move forward, a member of The Canine Condition Foundation will contact you to schedule a phone interview. If your application is approved following the phone interview and your meet-and-greet with the dog, you will be asked to provide a copy of your government-issued photo ID, sign the adoption agreement, and pay the adoption fee before finalizing the adoption.
