• Thank you for considering donating a pet to Pets for Vets®.

Pets for Vets will consider pets that meet the following criteria:

    1. Between the ages of 2-5 years old
    2. Not likely to exhibit resource guarding or aggressive behaviors towards humans, other animals, food, and/or toys
    3. Preference for the company of humans and seek out social contact
    4. Have a low to medium energy level
    5. Non-reactive on leash
    6. Low prey-drive

If you feel the pet meets our criteria please submit the form for consideration. This form helps us learn about the pet's medical history, behavior, personality, and current needs. The form requests detailed information such as veterinary records, photos, and links to short videos, for us to assess if the pet is the right fit for the program. You can save your progress and return later if needed.

For videos, please provide viewable links (YouTube, Vimeo, Google Drive, or Dropbox) and make sure anyone with the link can view them. For safety, please do not stage or force any interaction that could be stressful or unsafe for your dog, another animal, a child, or another person.

Please answer all questions as accurately as possible. Submission of this form does not guarantee acceptance into the Pets for Vets program.
