  • Continued unsafe or disruptive behaviour may result in early pick up, suggestion of an alternative program/additional support, or removal from program. Campers may return where appropriate accommodations or supports can be implemented, in collaboration with the family, camper, and camp staff.

    I will disclose all the necessary information about my child’s needs and abilities on the registration form. I am aware that my child may be removed from the program if any of the following situations occur that pull staff out of ratio and/or result in safety concerns which could include:
    • Aggression or violence towards self or others
    • Fleeing, hiding, or leaving program space
    • Ongoing refusal to follow the directions of staff
    • Destruction of YMCA property or others’ personal belongings
    • Lewd or sexual behaviour
    • Language that does not promote a respectful or inclusive environment

    I understand a parent or guardian must be available to pick up my child if requested by YMCA staff.
  • Registered Activities
    By registering my child(ren) for the YMCA of Hamilton|Burlington|Brantford Day Camp I give consent for them to participate in all activities listed on the Day Camp website. I also agree to follow the instructions located within.

    Authorized Pick Ups Only
    Only individuals listed as authorized pick-ups or emergency contacts (16+) during the registration process will be permitted to sign participants in and out. Authorized pick-ups, including parents/caregivers, will be required to show valid, government issued photo ID daily. For the safety of everyone involved, parents/caregivers and authorized individuals are asked to remain at the sign-in area and are not permitted inside the program.

    Sign Out
    I understand that I or anyone I send (16+) will be required to show valid government issued photo identification for the purposes of signing out my child(ren). Individuals signing out must be listed as an authorized pick-up or emergency contact on the registration form in order to sign out a camper.

    Medication
    As the YMCA must be notified in advance of camper medication, I agree to complete and sign all necessary medical forms prior to my child's registered week of camp. I understand that I should only send enough medication for the week. I will ensure staff are aware of the medication(s) my child may/will take at the point of sign in.

    Valuables and Personal Belongings
    I understand that valuables such as collectable cards, tablets, handheld video games, cell phones, etc. are not permitted at camp. All necessary items will be clearly labelled with my child’s first and last name. I understand that the YMCA is not responsible for the loss or damage of my child’s belongings at camp.

    Custody
    I understand the YMCA must be notified of any legal custody arrangements prior to my child's registered week of camp. I will ensure the YMCA receives a copy of the court order. I understand that if I do not provide documentation, the YMCA will release my child(ren) to any individual listed on the camp registration form (Primary/Secondary Contacts, Emergency Contacts, and/or Authorized Pickups).

    Code of Conduct
    As the safety of each individual is of the utmost importance to the YMCA, my child and I recognize a personal responsibility to learn and follow safety and other rules established by YMCA staff at all times. My child and I understand that any behaviour that places my child or others at risk physically or emotionally (including running away, physical behaviour, destructive behaviour, harassment and/or bullying, etc.) may result in immediate dismissal or removal from the program. I understand no refund will be granted for dismissal of my child before the end of the registered camp session or week.

    I have disclosed all the necessary information about my child’s needs and abilities on the registration form. I am aware that my child may be removed from the program if I falsify and/or fail to share the requested information with the YMCA. I understand a parent or guardian must be available to pick up my child if requested by YMCA staff.

    Day Camp Authorization
    • I have read the policies and procedures above and in the Day Camp Field Guide and agree to comply as specified.
    • I confirm that my child is able to participate in a full range of activities as described in the Day Camp Field Guide.
    • I understand that my child is required to bring and apply their own sunscreen and bug spray (optional).
    • I will not hold the YMCA responsible for lost or stolen items.
    • I give consent that medical treatment be given in the event of an emergency.
    • I acknowledge that in the event of camp cancellation, camper illness or injury, or behavioral concerns, a parent or authorized pickup or emergency contact must be available to pick up my child from camp at the request of YMCA staff.
    • I will not hold the YMCA, its staff or volunteers responsible for accidents which may occur.
    • I understand the legal obligation of the staff to report any suspected abuse.
    • I understand that the YMCA may decline a child due to physical or verbal aggression towards staff or other children, or if the safety of the child and/or others is at risk.
