    Session 1 starts September 21 | Session 2 starts February

  • All NEW clients are required to create a Court Reserve account and create their Payment Profile. If you are a returning client, you should already have a Court Reserve account, but must create a Payment Profile.

    Placement into a class is contingent upon being registered for Court Reserve and having a Payment Profile.

    You will NOT be placed unless this is completed.

    A deposit ($150) is required to submit this form to secure a spot in a class and will go towards your class balance when registered.

    Deposit is refunded if we are unable to place you in a class. Deposit is non-refundable once you are placed for your class.

  • Please note that all credit card transactions are subject to a 3% processing fee, with the exception of the initial form deposit. Although a credit card is required to set up a valid payment profile in the member portal, your card will only be charged if you select it as your primary payment method or if an account balance remains unpaid past the payment due date.

  • PARENT OR LEGAL GUARDIAN ACKNOWLEDGEMENT (IF UNDER THE AGE OF 18)

    I represent and warrant that I am the parent or legal guardian of the above applicant, the individual who signed the foregoing Release (“Minor”) and that I have received, read, and understood the foregoing Release. I fully consent to and voluntarily authorize the Minor to execute said Release (or, if applicable, have voluntarily executed said Release on Minor’s behalf). I acknowledge and agree that all representations, consents, agreements, grants, waivers, authorizations, indemnifications and releases herein shall be regarded as made by me on behalf of the Minor and shall be binding on me and the Minor.

    Furthermore, in consideration of Releasees possibly including me and/or Minor in the Event, I hereby agree to be bound by and to perform all of the terms and conditions of the foregoing Release (including, without limitation, the provisions regarding release of all claims), as such terms and conditions may relate to my participation and/or the participation of the Minor in the Event, if any.

    Once the form is submitted, you will be asked to create a CourtReserve profile. If you are a NEW customer, you must fill out the profile for your child and the payment profile must have a working credit card listed.
