Young Adult Field Day Participation Waiver and Release of Liability
Assumption of Risk
I understand that participation in the United Nations Church Intl Young Adult Field Day involves recreational and physical activities that may include running, relay races, team games, outdoor activities, and other physical challenges. I acknowledge that these activities involve inherent risks, including but not limited to slips, falls, collisions, strains, sprains, and other injuries.
I voluntarily choose to participate in this event and assume all risks associated with my participation.
Release of Liability
In consideration of being allowed to participate in the United Nations Church Intl Young Adult Field Day, I, on behalf of myself, my heirs, personal representatives, and assigns, hereby release, waive, and discharge United Nations Church Intl, its pastors, leaders, volunteers, staff members, and representatives from any and all claims, demands, causes of action, liabilities, damages, or expenses arising out of or related to my participation in the event, except where caused by gross negligence or intentional misconduct to the extent permitted by applicable law.
Medical Authorization
I certify that I am physically able to participate in the activities offered during the event.
In the event of an emergency, I authorize United Nations Church Intl to obtain emergency medical treatment for me if I am unable to do so myself. I understand that I am responsible for any medical expenses incurred.
Code of Conduct
I agree to conduct myself respectfully and responsibly throughout the event. I understand that United Nations Church Intl reserves the right to remove any participant whose behavior is disruptive, unsafe, or inappropriate without refund or compensation.
Photo and Video Release
I grant United Nations Church Intl permission to photograph, video record, and otherwise capture my image and likeness during the event. I understand these images and recordings may be used for church publications, social media, websites, promotional materials, and other ministry purposes without compensation.
