  • INFORMED CONSENT

    I hereby give my approval for my child’s participation in any and all activities prepared by Ridgeline Volleyball Club during the selected clinic/event. I assume all risk and hazards incidental to the conduct of these activities and release, absolve, and hold harmless Ridgeline Volleyball Club and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from clinic sessions.

    In the case of injury to said child, I hereby waive all claims against Ridgeline Volleyball Club, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. I acknowledge that there is a risk of injury inherent in all sports activities, including volleyball. These injuries may include, but are not limited to, fractures, paralysis, or death.

  • Photo/Video Release and Marketing Use

    I understand that photos and/or videos may be taken during Ridgeline Volleyball practices, training sessions, events, and related activities. These photos and videos may include my athlete and may be used by Ridgeline Volleyball for marketing, promotional, social media, website, and informational purposes.

    I understand that Ridgeline Volleyball will make reasonable efforts to use photos and videos respectfully and appropriately. Athlete names will not be used in connection with photos or videos without additional permission, unless already provided for team, roster, or program-related purposes.

    I understand that I may opt out of photo/video use for my athlete by notifying Ridgeline Volleyball in writing before the program begins. If I choose to opt out, I understand that Ridgeline Volleyball will make reasonable efforts to avoid using identifiable images or videos of my athlete in future marketing materials.
