Adopt-a-Pet Art Camp Registration Form

Please read ALL information before registering.

All camps/classes have limited spots, first come first served, and no holds without payment. I cannot guarantee your child's spot until both the registration form and payment is received. Thank you for understanding. Students with a Step Up scholarship will be provided a detailed invoice to submit for reimbursement.

The Adopt-a-Pet Art Camp fee is $60 and can be paid via Venmo or PayPal: bayouartstudio OR bayouartstudio@gmail.com

All camps and classes are held at Bayou Creative Arts Studio located at:

3127 West International Speedway Blvd, Daytona Beach, FL 32124

Camps are planned as drop off events, but parents/guardians are welcome to remain in the lobby where there is seating and a full view of their child at all times. Doors will remain locked during all camps and classes and only opened to authorized people.

IMPORTANT: Camps and classes are non-refundable, but if an emergency or illness prevents your child from attending I will issue a credit to be used for future classes, camps, or events.

I understand that the Adopt-a-Pet Art Camp is scheduled for Friday, July 24 and from 9AM-12PM and that camp is non-refundable. If my child is unable to attend, a credit will be issued.

Consent for Social Media: Photos/videos may be taken of your child during sewing classes, camps, and workshops. The photos/videos with the child's first initial may be used on Bayou Creative Arts Studio’s website or social networks to which I, Jessica Fulton, own and have the rights to post the photos/videos on my business website or social networks.

Hold Harmless Agreement: I understand the instructors of Bayou Creative Arts Studio will do their best to prevent any accidents from occurring. However, I, the undersigned, agree to hold the instructors harmless and not liable for any accident that may occur to my child(ren) while attending sewing classes, camps, and workshops held by the instructors.
