Consent to Use AI-Assisted Clinical Documentation (Heidi Health)
Please read the information below and indicate your consent regarding the use of HEIDI, an AI tool used to assist with session note documentation. Your participation is optional and will not affect your care.
To help me focus more on you during our sessions and spend less time writing notes, I use an AI-assisted clinical documentation tool called Heidi Health. Heidi listens to our session and generates a draft of my clinical notes. I carefully review, edit, and approve all notes before they become part of your clinical record. The AI does not make clinical decisions or replace my professional judgement.
What this means:
• Heidi Health will process the conversation during your appointment to assist with drafting clinical notes.
• I remain fully responsible for all clinical documentation and will review and edit every note before it is saved.
• The AI is used only to assist with documentation and does not provide treatment, diagnosis, or clinical recommendations.
• Your information is handled in accordance with applicable Australian privacy laws and Heidi Health's security and privacy standards.
Your choice:
• Using Heidi Health is completely optional.
• You may choose not to consent.
• Declining will not affect the care you receive.
• If you decline, I will document the session using my usual manual note-taking process.
• You may withdraw your consent at any time before or during a session.
