1) Purpose & Scope
This Agreement sets expectations for a professional consultation relationship focused on
the consultee's use of standard EMDR therapy with clients, including feedback on clinical
decision-making, fidelity to the eight phases/three-pronged approach, and appropriate
modifications when necessary to support safe, effective care. Consultation is not
supervision and does not create an employment relationship. The consultee retains full
responsibility and autonomy for all clinical decisions and client care.

2) Definitions
Consultant:
Bonita Ashe (Approved Consultant per EMDRIA). The person providing the
consultation service.
Consultee:
The person receiving consultation.
Consultation:
A structured, consultee-centered process in which the consultant reviews
case material and provides feedback on the consultee's use of standard EMDR therapy.
Examples of clinical work (e.g., recordings, near-verbatim transcripts, case presentation
forms) are part of the process.
3) Eligibility & Credentials
The consultee affirms that they are credentialed or licensed to practice psychotherapy or
are in the process of meeting licensure requirements, and if unlicensed, are receiving
regular clinical supervision from a licensed clinician.
If unlicensed, the consultee agrees to provide their supervisor's name, email, and phone and
authorize coordination as needed to ensure ethical and clinically sound guidance.

5) Consultation Pathways & Hours
EMDR Basic Training consultation (10 hours required by training program)
6) Nature of Services & Limits (Consultation vs. Supervision)
Consultation focuses on mastery and integration of standard EMDR therapy in practice. The
consultant evaluates the consultee's fidelity to the eight phases/three-pronged approach
and awareness of when modifications are indicated. Consultation is not clinical supervision
and does not substitute for supervision required by licensing boards. The consultee remains
fully responsible for client care and legal/ethical compliance.
7) Group Consultation Parameters
EMDR basic training consultation – Group consultation size for basic training
cannot exceed 8 consultees(1:8). When group time is not equally divided among
consultees, each participant is expected to present case material, so that all
consultees in the group both demonstrate and observe their peer's demonstration
of standard EMDR therapy. No more than 2 hours of consultation can be provided
at once or per week.
Individual Consultation will only be provided 1:1 with the consultee and consultant.
8) Attendance & Scheduling
Group consultation (RSVP policy): Groups depend on RSVPs; if you RSVP,
attendance is expected. If you are unable to attend, payment is still due unless
otherwise agreed upon by the consultant. You may withdraw from a group with two
(2) weeks' notice.

Individual consultation (cancellation policy): If you have scheduled an individual consultation, attendance is expected. You may cancel without charge with 24 business hours' notice. Otherwise, payment is still due unless otherwise agreed upon.
A consultee cannot complete more than 2 hours of group per week to ensure adequate time to practice with real clients. Consultations must be scheduled to meet the consultee where they are at developmentally and to ensure they have clients and time to practice. You can only log up to 6 hours of consultation for a consultee prior to weekend 2 (Part 2 of 2). You can track your hours here on the Trainee Records section of the website.
It is vital you come prepared with items for consultation, as this is your EMDR learning process. Avoid coming to consultation and saying, "I don't have anything." If you just recently attended the Basic EMDR training you can bring insights and thoughts, and questions from there! If you have not started EMDR therapy with a client, bring potential cases to staff for readiness and appropriateness.
Please be respectful of group members. Do not speak over one another. Offer supportive feedback and ask questions as they relate to other group members' topics. Please be mindful of any background noises in your environment. Mute your side if you are typing, shuffling papers, outside on a windy day, standing in a chicken coop, hanging out on train tracks, etc.
Please do NOT attend consultation while you are driving. You will be asked to leave as this is not safe.
Plan to bring a case to review. For each case you will be asked to share basic demographics (no PHI please), cultural considerations (adaptive and adverse), history, and current presenting info.
9) Fees and Payment Terms
Fees: $42 per hour (discounted rate for HAP Basic Training consultation) otherwise $65.
Invoicing & Due Date: Payment is due upon scheduling unless prepayments or other arrangements are made in advance.
10) Documentation, Recordkeeping & Letters
The consultee will maintain their own records to track their consultation hours, case conceptualization forms, videos, recordings, and/or near verbatims. Consultees will ensure they have permission from their client to meet HIPAA and will only share their screen and remove identifying PHI.
All consultees (Basic Training, Certification, and CITs) are required to complete and share behavioral samples to aid in feedback, recommendations and to demonstrate proficiency. Below is a list of materials that may be provided. You and your consultant will identify preferred materials.

The consultant will track and document consultation hours (individual and group) and retain records for five (5) years from the date consultation concludes. They will also document completion of the behavioral samples and feedback provided. Consultees will keep a record of behavioral samples completed.
11) Confidentiality & Case Materials
Both parties will maintain confidentiality consistent with law and professional ethics. Case materials must be de-identified prior to sharing. When using recordings for consultation, the consultee will obtain written client consent using appropriate consent for recording form. Consultees will no transmit documents with client info to the consultant and will only share their screen and remove identifying PHI.
All information shared during EMDR group consultation is considered confidential and is to be treated with the same care and respect as client information in clinical practice. Participants (Consultant & Consultees) agree not to disclose any identifying details shared by others and to de-identify client material when presenting cases.
While efforts are made to maintain privacy, confidentiality cannot be absolutely guaranteed in a group setting, and all members share responsibility for protecting the integrity of the consultation space. Limits to confidentiality include circumstances required by law or ethical mandate, such as risk of harm to self or others, abuse or neglect, or court order. Participation in the group indicates agreement to uphold these expectations.
12) Coordination with Clinical Supervisor (If Unlicensed)
If the consultee is unlicensed, they authorize the consultant to coordinate with their licensed clinical supervisor as needed and agree to provide accurate supervisor contact information (above in Clinical Supervisor Section).
13) Communication With Other Consultants (If Applicable)
Consultees may work with additional consultants to address specific learning needs. For Trauma Recovery/HAP Basic Training, no more than
two
faculty consultants may be involved at one time. With the consultee's consent, consultants may communicate to coordinate guidance and accurately track hours toward certification.

Consultees must notify Bonita Ashe before adding another consultant. Bonita Ashe reserves the right to end the consultation relationship if involving additional consultants is not appropriate, including concerns about developmental readiness, rushing the process, limited practice, or difficulty using the standard protocol. These examples are not exhaustive.
14) Technology, Security & Tele-consultation
Consultation may occur in person or via secure video/phone. The consultee is responsible for using HIPAA-aligned platforms for transmitting any PHI, de-identifying materials whenever possible, and complying with their jurisdiction's telehealth and data-privacy rules.
15) Clarification of Consultant Role and Capacity
Clarification of Roles and Capacity
By signing below, you acknowledge and understand that I am providing this consultation in my capacity as a Trauma Recovery/HAP-approved Facilitator and Consultant, fulfilling the required consultation hours for EMDR Basic Training under Trauma Recovery/HAP's training program. This consultation is separate and independent from my private practice, Five Stones Therapy, LLC. Five Stones Therapy, LLC is not involved in, and bears no responsibility for, the consultation provided under this Agreement.
16) Acknowledgment of Potential Role-Related Influence
I understand that Bonita Ashe may hold inherent role-related influences regarding EMDR training, advanced trainings, and disaster response as a result of their role with Trauma Recovery Humanitarian Assistance Programs (Trauma Recovery/HAP). Should any concerns arise related to these role-related influences, I agree to discuss them directly with her. I also understand that all consultation services provided under Five Stones Therapy, LLC are separate from, and not affiliated with, Trauma Recovery/HAP in any capacity.
17) Term, Withdrawal & Termination
This Agreement remains in effect until terminated by either party. Either party may terminate at any time with written notice. Group withdrawal requires two (2) weeks' notice (see §7). If needed Basic Training Consultee will use the Trauma Recovery/HAP Consultant Directory to find a new consultant. Certification and CITs can also use this resource to locate a new consultant or they can use EMDRIA.ORG.
18) Rights and Responsibilities
The consultant and consultee agree to engage in EMDR consultation with mutual respect, professionalism, and a shared commitment to ethical practice and clinical growth. The consultee has the right to receive guidance, feedback, and support consistent with EMDR

standards, and is responsible for presenting clinical material accurately, maintaining client confidentiality, and practicing within their scope of competence. The consultant has the right to set clear expectations, provide constructive feedback, and maintain appropriate boundaries, and is responsible for offering consultation that aligns with EMDR protocols, ethical guidelines, and current best practices. Both parties agree to maintain confidentiality, communicate openly, address any concerns in a timely manner, and uphold all applicable legal and professional standards throughout the consultation relationship.
19) Dispute Resolution
In the event of a concern, dispute, or grievance related to EMDR consultation or training, the consultee is encouraged to first address the issue directly with the consultant in a timely and professional manner in an effort to reach informal resolution. If the matter cannot be resolved collaboratively, the consultee may submit a formal written complaint outlining the nature of the concern and desired resolution. For issues related to EMDR Basic Training, participants may follow the grievance procedures outlined by EMDR Humanitarian Assistance Programs (EMDR HAP) and/or the EMDR International Association (EMDRIA), the accrediting body. EMDRIA provides a formal grievance process in which complaints can be submitted in writing and reviewed for appropriate action, with efforts made to maintain confidentiality, ensure fair review, and provide a response or update within a reasonable timeframe. Participants may contact EMDRIA directly (e.g., via info@emdria.org) to initiate a formal complaint if concerns remain unresolved at the training or consultation level. All parties agree to engage in good faith, uphold professional standards, and avoid retaliation throughout the grievance process.
20) Acknowledgments & Consents
No supervision relationship exists between consultant and consultee.
The consultee is currently completing the EMDR Therapy Basic Training requirements and asserts they are credentialed/licensed to practice psychotherapy; if unlicensed, they are receiving regular clinical supervision.
If unlicensed, you agree that you have your supervisor's consent to receive EMDR Consultation, and they agree to coordinate with your EMDRIA Approved Consultant.
Electronic signature consent: Typing their name serves as an electronic signature and indicates agreement with this consultation agreement.
21) Entire Agreement; Amendments
This Agreement represents the entire understanding between the parties and supersedes prior discussions. Any amendment must be in writing and signed by both parties.
