THE GRIND LAB TAMPA AFTER-SCHOOL PROGRAM
Liability • Transportation • Recurring Payment Authorization
I, the undersigned parent/legal guardian, voluntarily give permission for my child to participate in The Grind Lab Tampa After-School Program, including athletic training, baseball/softball activities, strength and conditioning, speed and agility training, fitness activities, academic/study periods, recreational activities, and other supervised program activities. I understand that participation in athletic and physical activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, equipment-related injuries, strains, sprains, fractures, and other injuries, and I voluntarily assume the ordinary risks associated with my child's participation. To the fullest extent permitted by Florida law, I agree to release and hold harmless The Grind Lab Tampa, Albury Athletics, their owners, directors, employees, coaches, trainers, volunteers, contractors, agents, affiliates, and representatives from claims arising from the ordinary and inherent risks associated with my child's voluntary participation, except for rights or liabilities that cannot legally be waived. In the event of illness, injury, or emergency and I cannot be reached, I authorize The Grind Lab Tampa staff to contact emergency medical services and obtain reasonably necessary emergency care for my child, and I understand that I am responsible for associated medical expenses except where otherwise required by law. I further authorize The Grind Lab Tampa/Albury Athletics and its authorized drivers and staff to transport my child from their approved school or pick-up location to The Grind Lab Tampa facility and, when applicable and communicated to me, to approved program-related activities. I understand that motor vehicle transportation involves inherent risks, including traffic accidents, delays, mechanical issues, actions of other motorists, and circumstances outside the reasonable control of The Grind Lab Tampa/Albury Athletics, and I voluntarily authorize such transportation. I understand that my child is expected to follow all transportation safety rules, remain properly seated, wear a seat belt as required, behave appropriately, and follow all instructions provided by authorized drivers and staff. I agree to notify the program when my child will be absent or does not require scheduled transportation and understand that my child will only be released to me or another person I have authorized unless other written authorization is provided. I further understand and agree that the credit or debit card I am using to make today's payment will be the payment method kept on file for my child's recurring monthly After-School Program payments, and by signing this agreement I expressly authorize The Grind Lab Tampa/Albury Athletics to charge that payment method each month according to my agreed monthly billing schedule until the subscription is properly canceled or an approved replacement payment method is provided. If I wish to cancel my subscription and stop future recurring charges, I understand that I must submit my cancellation request by email to coacht@joinalburyathletics.com no later than 12:00 PM (noon) on the Sunday prior to my next scheduled charge. I understand that a cancellation request received after this deadline may not prevent the next scheduled payment from being processed and that cancellation does not automatically entitle me to a refund of payments already properly processed or amounts already due under my program agreement. By signing below, I acknowledge that I am the parent/legal guardian or otherwise legally authorized to sign on behalf of the child; that I have read and understand this entire agreement; that I have had the opportunity to ask questions; and that I voluntarily authorize my child's participation, transportation, selected media permission, emergency assistance, and recurring monthly payment arrangement as stated above.
