Event: Comprehensive Youth Development Adult Field Day

I acknowledge that participation in the Comprehensive Youth Development Adult Field Day is voluntary and may involve physical activities, including but not limited to running, jumping, relay races, lawn games, recreational sports, and other group activities.
I understand that participation carries inherent risks, including but not limited to falls, collisions with other participants or equipment, weather-related conditions, dehydration, muscle strains, sprains, broken bones, and other injuries. I knowingly and voluntarily assume all such risks, whether known or unknown.

To the fullest extent permitted by law, I release, waive, and discharge Comprehensive Youth Development, its officers, directors, employees, volunteers, sponsors, partners, agents, and representatives from any and all claims, demands, causes of action, liabilities, damages, costs, or expenses arising out of or related to my participation in the event, including any injury, illness, property damage, or loss, except to the extent caused by gross negligence or willful misconduct.

I certify that I am physically able to participate in the activities offered during the event. I understand that Comprehensive Youth Development will not provide medical insurance for participants.

In the event of an emergency, I authorize event staff to obtain emergency medical treatment on my behalf if I am unable to do so. I understand that I am responsible for any medical expenses incurred.

I agree to:

• Follow all instructions provided by event staff and volunteers.
• Participate in a respectful and sportsmanlike manner.
• Refrain from behavior that could endanger myself or others.
• Understand that organizers may remove me from the event without refund or compensation if I engage in unsafe, disruptive, or inappropriate conduct.

I grant Comprehensive Youth Development permission to photograph, film, or otherwise record me during the event and to use my likeness for promotional, educational, fundraising, and marketing purposes, in print, online, and on social media, without compensation.

I certify that:

• I am at least 18 years of age.
• I have carefully read this Waiver and Release of Liability.
• I understand its contents and sign it voluntarily.
• I understand that by signing this document I am giving up certain legal rights.
