  • 1. Event Participation Permission

    I understand and agree that my child has permission to participate in the Ulta Beauty and YWCA Metro St. Louis PRETTY EMPOWERED Youth Event held on Saturday, October 24, 2026. The Event is sponsored by, or in part by, YWCA Metro St. Louis, YWCA USA and Ulta Beauty.

    I understand that my child’s participation in the Event is voluntary. I elect for my child to participate and agree that my child will follow all Event rules and transportation guidelines.

  • 2. Release of Liability for Participation in the Event

    I voluntarily agree to release, indemnify, and hold harmless YWCA Metro St. Louis, its employees, Board members, agents, volunteers, and event partners from any claims, liabilities, costs, or expenses arising from or connected to my child’s attendance or participation in the Event.

    I understand that participation may involve risks, including risks that may not be foreseeable, and I voluntarily assume these risks.

  • 3. Transportation Release & Agreement

    YWCA Metro St. Louis will provide transportation for my child to and from the PRETTY EMPOWERED Youth Event on Saturday, October 24, 2026.

    I agree to drop off my child at the designated pick-up location no later than 8:00 AM on the day of the Event. I understand that YWCA Metro St. Louis will transport my child to the Event and return my child to the designated pick-up location at approximately 2:00 PM.

    I understand that transportation involves inherent risks, including risks that may not be foreseeable. By permitting my child to participate in YWCA-provided transportation, I voluntarily assume these risks and agree that my child will follow all transportation rules and instructions provided by YWCA staff.

    I release, waive, indemnify, and hold harmless YWCA Metro St. Louis, its employees, Board members, agents, volunteers, and transportation partners from any claims, liabilities, costs, or expenses arising out of or related to my child’s transportation to or from the Event, except where prohibited by law.

  • 4. Emergency Medical Treatment Consent

    In the event of injury or serious illness, I give permission for YWCA Metro St. Louis to obtain medical treatment for my child. I authorize YWCA Metro St. Louis to secure services from a physician, healthcare provider, hospital, or emergency personnel as needed. I understand that I am responsible for any medical expenses incurred.

    I understand that YWCA Metro St. Louis may not maintain insurance coverage for injuries or damages related to my child’s participation. I certify that I have adequate insurance coverage or agree to personally assume responsibility for related costs.

    I give YWCA Metro St. Louis, its representatives, and authorized partners permission to photograph, record, reproduce, publish, display, distribute, and use photographs, videos, and recordings of my child taken during the Event.

    I understand these materials may be used for promotional, educational, informational, or organizational purposes in print, digital, online, social media, and other media formats now known or developed in the future.

    I understand that permission does not include use intended to portray my child in a misleading, inappropriate, or disparaging manner.

    I agree that if any portion of this Agreement is found to be unenforceable, the remaining portions will remain in effect. I agree that any legal action related to this Agreement or the Event will be handled in the State of Missouri.
