Parent/Guardian Consent
Parent/Guardian Consent I, the undersigned parent or legal guardian of the above-named child, grant permission for my child to participate in the CKIA Kingdom Kids Conference hosted during Holy Conference. I understand that my child's participation is voluntary and that I have carefully read and agree to the terms outlined in this waiver.
Medical Authorization
In the event of an accident, injury, or medical emergency, I authorize the conference staff, volunteers, and designated leaders to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that every reasonable effort will be made to contact me prior to any non-emergency medical treatment. I agree to assume full responsibility for all medical expenses incurred on behalf of my child.
Assumption of Risk & Release of Liability
I acknowledge that participation in children's ministry activities, including games, crafts, worship services, recreation, group activities, transportation (if provided), and other conference events, involves inherent risks that cannot be completely eliminated. On behalf of myself and my child, I voluntarily assume all such risks. To the fullest extent permitted by law, I release, waive, and hold harmless CKIA Kingdom Kids Conference, Holy Conference, Love Fellowship Tabernacle, its pastors, officers, employees, volunteers, ministry leaders, sponsors, and affiliates from any claims, demands, causes of action, injuries, damages, losses, or liabilities arising from my child's participation, except where caused by gross negligence or intentional misconduct
Behavior Expectations
I understand that all participants are expected to demonstrate Christ-like conduct, show respect for others, and follow the directions of conference staff and volunteers. If my child's behavior becomes disruptive, unsafe, or inappropriate, I understand that I may be contacted and asked to pick up my child before the conclusion of the conference.
Photo, Video & Media Release
I grant permission to CKIA Kingdom Kids Conference, Holy Conference, and Love Fellowship Tabernacle to photograph, videotape, livestream, or otherwise record my child during conference activities. I understand these images or recordings may be used without compensation for purposes including, but not limited to: Church publications, Church website, Social media platforms ,Promotional materials , Livestreams and broadcasts , Future conference promotions , Ministry presentations I understand that names will not be published without additional permission unless required for ministry recognition. I release the conference and church from any claims related to the use of these photographs, videos, or recordings.
Transportation Permission (If Applicable)
I authorize my child to participate in transportation provided by conference staff or approved volunteers during conference-related activities. My child will not require transportation provided by the conference.
Personal Property
I understand that CKIA Kingdom Kids Conference, Holy Conference will not be responsible for lost, stolen, or damaged personal belongings brought to the conference.
Parent/Guardian Agreement
By signing below, I acknowledge that: I have carefully read and understand this waiver. I voluntarily agree to all terms and conditions contained herein. I certify that I am the parent or legal guardian of the participating child. The information provided on this form is true and complete to the best of my knowledge. I understand that this agreement remains effective for the duration of the CKIA Kingdom Kids Conference unless revoked in writing prior to my child's participation.
