Event Name: Cowichan River Float (Tube Rentals and shuttle through The Tube Shack)

Event Date(s): Saturday, August 15th

Event Location: Bethel Church, Tube Shack, and Cowichan River

Cost: $25 (includes tube rental and shuttle)

Please pay via e-transfer to finances@bethelonline.ca and write in the notes Bethel Youth Event.

Event Details
- Meet at Bethel Church at 11:45 AM
- Arrive at the Tube Shack at 1:00 PM
- Float Down the River
- Leave the Tube Shack at approx. 5:00 PM
- Arrive at Bethel at 6:00 PM (This is an estimation, we will communicate when we are leaving the Tube Shack)
Please bring the following:
- Towel
- Water Bottle
- a bag that can get wet (to put drinking water and shoes in for the float),
- snacks
- sunscreen
- modest swimsuit
- flip flops or sandals that can get wet
- lunch to eat on the way if not eating before leaving
When this form is complete, you will be redirected to fill our the Tube Shack Waiver form that must be filled out.

Purpose of Form

This form allows your child to participate in a church-sponsored off-site event and ensures parents/guardians understand, acknowledge, and accept the risks associated with travel and event participation.
Consent to Particiapte

I, the undersigned parent or legal guardian, hereby give permission for my child to participate in the off-site event organized by Bethel Church, including all scheduled activities and transportation to and from the event.
Acknowledgement of Potential Risks

I understand that participation in off-site events may involve risks, including but not limited to:

General Risks
- Trips, slips, and falls
- Minor injuries such as sprains, cuts, bruises
- Contact with environmental hazards (uneven ground, weather, natural surroundings)
- Loss or damage of personal belongings
Transportation-Related Risks
- Motor vehicle collisions or breakdowns
- Delays or unexpected route changes
- Injuries while entering/exiting vehicles
Activity-Specific Risks
- drowning
- insect bites, allergice reactions, rough terrain
- use of river tube
Health-Related Risks
- Allergic reactions
- Illness during the event
- Exposure to contagious conditions
- Medical emergencies requiring first aid or medical support
Interpersonal or Behaviour-Related Risks
- Accidental injury caused by another participant
- Behavioural issues requiring early pick-up
- Emotional stress or conflict with peers
Unforeseen Risks

I acknowledge that risks may exist that are not reasonably foreseeable or listed above.
Medical Consent
I authorize adult leaders of Bethel Church to:
- Provide first aid or basic medical care
- Seek emergency medical treatment if deemed necessary
- Transport or arrange transportation for my child to a medical facility
I assume all responsibility for medical costs that may result.
Any allergies, medical conditions, or behaviours we should know about?
"Participation in this activity requires basic swimming ability. Unfortunately, students who are not able to swim will not be able to join for safety reasons."
Release of Liability
In consideration for allowing my child to participate, I hereby:
- Release and forever discharge Bethel Church, its pastors, employees, volunteers, representatives, and event leaders from any and all liability for injury, illness, accident, property damage, or death that may occur during or as a result of this event, except where prohibited by law.
- Agree to indemnify and hold harmless Bethel Church from any claims or demands arising from my child’s participation, whether caused by negligence or otherwise, to the extent permitted by law.
Behaviour Expectations
I understand that my child must follow all church rules, event guidelines, and instructions from leaders. I agree that if behavioural issues arise, my child may be temporarily removed from activities or sent home at my expense.
