I am giving my two weeks notice per my original contract with Tumble Time Gymnastics LLC. I acknowledge that by signing and submitting this request I am releasing the reserved class time that my child currently holds and that if I choose to, I will have to re-register for classes to resume services. I understand that my child will still be able to attend for the next two weeks and that appropriate charges per contract for those two weeks will still be charged, if applicable.
