IRS 501 (c) 3 letter (Required if the applicant organization is a non profit)
Financial Statements or a single audit if applicable prepared by Certified Public Accountant (CPA), The submission must provide information covering the year ending December 31, 2024 or later fiscal year.
A copy of the organization's Code of Conduct Policy, and dated within the last five (5) years.
A current Board of Directors list including dates when current terms end, identification of current officers, and identification of persons(s) who are homeless/formerly homeless.
Evidence that the organization is eligible to contract with the federal government to spend federal funds (available at www.sam.gov). Also called Summary Document that will provide information as proof of not being debarred.
Approved budget of the organization for the current fiscal year
