Please read this waiver carefully before participating in a GodBody Sunday workout.
Health and Voluntary Participation
Physical activity carries risks including falls, strains, sprains, illness, serious injury, disability, or death. By signing below, you confirm you are physically able to participate, have consulted a healthcare professional if needed, and will stop if you feel concerning symptoms.
Assumption of Risk and Release
To the fullest extent permitted by law, you knowingly assume all risks and release Divine Love Health Center, GodBody Sunday, their owners, instructors, employees, volunteers, contractors, and event partners from any claims or damages arising from participation, except where prohibited by law. This does not release liability for intentional misconduct or gross negligence where prohibited.
Personal Responsibility and Conduct
You agree to follow instructor directions, use equipment safely, respect other participants, disclose any limitations, and are responsible for your personal belongings.
Emergency Care
You authorize reasonable emergency medical assistance if needed and accept responsibility for any medical costs incurred.
