Event & Attendance: I understand that I must arrive at MOSI at 9:00 AM to set up on the day of the Event
Participation Agreement
I understand that submitting this form does not guarantee a spot at this event. Participation is subject to review and approval by MOSI
I understand that I must provide a current Certificate of Insurance (COI) prior to participating in this event.
I understand that only prepackaged food items are permitted at this event. On-site cooking and food preparation are not allowed, and I agree to follow these requirements.
Certificate of Insurance Required

MOSI and Hillsborough County Board of County Commissioners requires vendors to provide a Certificate of Insurance with coverage of $1,000,000 per occurrence and $1,000,000 aggregate. The certificate must also include a waiver of subrogation in favor of MOSI and name the following entities as additional insureds:
