2.1. Acknowledgment of Accumulated Shedding & Density Changes* This field is required.
I understand that trapped shed hair will release during this service. I acknowledge that noticeable shedding, reduced hair density, temporary scalp sensitivity, uneven lengths, and the discovery of pre-existing matting or matted buildup (lint/product residue) are normal, predictable outcomes of removing bound hair structures.

2.2. Acknowledgment of Structural Weakness & Breakage Risks* This field is required.
I agree that all consultation fees, booking retainers, and completed services are strictly non-refundable. I acknowledge the strict 24-hour cancellation policy and understand that arriving over 15 minutes late incurs a $20 tardy fee, while no-shows incur a mandatory $35 fee. I accept that the salon is a cashless environment for deposits, and a standard $3 processing fee applies to digital card transactions.

2.3. Professional Stop Recommendation & Client Discretion* This field is required.
I agree that if my stylist determines during the service that my hair or scalp is experiencing excessive distress, severe weakness, or potential permanent damage, they reserve the right to recommend halting the service or altering the restoration plan. If I explicitly request that the stylist proceed against their professional recommendation, I voluntarily accept full responsibility for all resulting damage or hair loss.

3.1. Voluntary Assumption of Risk & Liability Release* This field is required.
I voluntarily assume all risks associated with braid takedown, loc comb-out, and detangling services. I hereby release and hold harmless Sapphire Loc & Twist Studio LLC, its owner (Lucy Sams), employees, independent contractors, and apprentices from any claims, suits, or financial damages related to foreseeable service outcomes—including accumulated shedding, reduced density, hair breakage, or altered length—except where explicitly prohibited by law or resulting from gross negligence or willful misconduct.
