Liability Waiver and Media Release
1. RELEASE OF LIABILITY
In consideration of being allowed to participate in programs, rehearsals, performances, or related activities organized by Beacon Arts Collective (BAC), I hereby release and hold harmless BAC, its directors, officers, volunteers, agents, employees, and 5 Bridges Church from any and all liability, claims, demands, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in any BAC-related activities, regardless of location.

2. ASSUMPTION OF RISK
I acknowledge that participation in performing arts activities involves certain risks, including, but not limited to, physical exertion, movement injuries, or unforeseen accidents. I voluntarily assume all such risks.

3. MEDICAL CONSENT
In the event of an emergency, I authorize BAC staff or volunteers to seek medical treatment deemed necessary for the participant. I understand that every effort will be made to contact a parent/guardian or emergency contact first. I agree to be financially responsible for any resulting medical expenses.

4. PHOTO/VIDEO RELEASE
I grant permission for BAC to photograph or video the participant during activities and use these images for promotional or educational purposes, including but not limited to social media, print, and website content. I understand no personal information will be shared without further consent.
