Official Participant Waiver, Release of Liability & Media Consent

I certify that I am at least eighteen (18) years of age.
If I am under eighteen (18) years of age, I understand that I may participate only with the written consent of my parent or legal guardian and approval from Tampa Bay Wiffleball Tournaments ("TBWT"). I acknowledge that TBWT reserves the right to deny participation to any minor at its sole discretion.

I understand that participation in a Tampa Bay Wiffleball Tournaments event involves inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. These risks include, but are not limited to:

• Being struck by a wiffle ball, bat, or other equipment.
• Slipping, tripping, falling, diving, or colliding with players or objects.
• Muscle strains, sprains, fractures, concussions, cuts, bruises, or other injuries.
• Heat exhaustion, dehydration, sun exposure, lightning, and other weather-related conditions.
• Uneven playing surfaces or other field hazards.
• Injuries resulting from the actions or negligence of other participants or spectators.

I voluntarily assume all risks associated with participating in TBWT events.

I certify that I am physically capable of participating in tournament play and have no medical condition that would make participation unsafe. I understand that it is my responsibility to consult with a physician if I have any concerns regarding my ability to participate.

In consideration for being permitted to participate in Tampa Bay Wiffleball Tournaments events, I voluntarily release, waive, discharge, and covenant not to sue Tampa Bay Wiffleball Tournaments, its owners, directors, tournament officials, volunteers, employees, sponsors, facility owners, municipalities, affiliates, and any other persons acting on behalf of the tournament from any and all claims, demands, damages, losses, liabilities, costs, or expenses arising from or related to my participation, including claims resulting from ordinary negligence, to the fullest extent permitted by applicable law.
This release applies to personal injury, illness, death, property damage, or any other loss arising from participation in tournament activities.

If I become injured or experience a medical emergency during a TBWT event, I authorize tournament staff to obtain emergency medical treatment deemed reasonably necessary. I understand that I am solely responsible for all medical expenses incurred.

I understand that I am responsible for ensuring that any equipment I use is safe and in proper playing condition. If tournament equipment is provided, I agree to use it responsibly and return it in substantially the same condition, excluding normal wear.

I grant Tampa Bay Wiffleball Tournaments the unrestricted right to photograph, film, livestream, and otherwise record my participation. I authorize TBWT to use my name, image, likeness, voice, and tournament results in promotional materials, websites, social media platforms, livestreams, printed publications, and future marketing without additional notice or compensation.

I agree to conduct myself in a respectful and sportsmanlike manner before, during, and after the event. I understand that the following may result in penalties, ejection, or removal from the tournament without refund:

• Unsportsmanlike conduct.
• Fighting or threatening behavior.
• Harassment or discrimination.
• Abuse toward officials, volunteers, opponents, or spectators.
• Intentional damage to tournament or facility property.
• Any conduct deemed unsafe or detrimental to the event.

Tournament officials reserve the right to make final decisions regarding participant conduct.

I agree not to participate while impaired by alcohol, illegal drugs, or any substance that creates an unsafe playing environment. Tournament officials may remove any participant believed to be impaired.

I understand that games may be delayed, suspended, shortened, rescheduled, or canceled due to weather, field conditions, emergencies, or other circumstances beyond the control of TBWT. I agree to abide by the tournament's published refund and cancellation policies.

I acknowledge that I have read, or have had the opportunity to read, the Official Tampa Bay Wiffleball Tournaments Playing Rules and Tournament Policies. I agree to follow all tournament rules and understand that tournament officials have final authority regarding rule interpretations and event administration.

I agree that my electronic signature or acceptance during online registration shall have the same legal force and effect as my handwritten signature.

This agreement shall be governed by and interpreted in accordance with the laws of the State of Florida. If any provision of this agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.

By signing below, I certify that:

• I have carefully read this Participant Waiver, Release of Liability, and Media Consent.
• I understand the risks associated with participation.
• I voluntarily agree to all terms contained in this agreement.
• I understand that this is a legally binding document.

I certify that I am the parent or legal guardian of the participant listed above. I have read this waiver, understand its contents, and voluntarily consent to the participant's involvement in Tampa Bay Wiffleball Tournaments events. On behalf of the minor and myself, I agree to the terms of this waiver.
