PLEASE SUBMIT THE FOLLOWING ITEMS WITH THIS APPLICATION:
1. Site Plans: Submit (1) electronic copy. Plans should include street names, lot dimensions, setbacks (front, side and rear), sidewalks/driveways, exact location, size & height of any existing building or structure, and exact location, size & height of any building or structure to be erected.
2. Photos: Attach photos of relevant sight from all angles.
3. FOR DRIVEWAYS ONLY: The Site Plan should include proposed entrance in relation to a road with a sight distance evaluation, vicinity map, a copy of the recorded plat and/or deed, and a copy of the estimate for the entrance installation.

It is the applicant's responsibility to contact the following companies and request inspections:

REQUIRED FOR ALL PROJECTS:
- Before you dig, YOU MUST call Miss Utility for markings at (800) 242-6342. NO EXCEPTIONS.
- An approved building permit from the Town of Galena must be visibly placed on the property until the project has been inspected and completed.
By signing and submitting this application, the applicant affirms that they are authorized to submit this permit application on behalf of the property owner, business, or entity identified herein, and certifies that all information, statements, and materials provided in connection with this application are true, accurate, and complete to the best of their knowledge and belief. The applicant understands that any false, misleading, or incomplete information may result in denial, suspension, or revocation of the permit and may subject the applicant to any applicable penalties under Town ordinances or other applicable law.
