    Supervision Service Agreement 

    Please read the following information carefully. After you have read this agreement please sign your name to accept the terms of this agreement. 

    Purpose, Goals and Objectives of Supervision 

    To monitor and promote the welfare of the supervisee’s clients. 
    To promote development of supervisee’s professional identity and competence. 
    To use the knowledge and wisdom within the group to support therapeutic practice. 
    Our group format is a closed ongoing group so that supervisee’s work with the same people to create stability and deepen the wisdom space. 

    Context and Content of Supervision 

    To provide group supervision on regular basis to focus upon frameworks, skills and creative therapy interventions. 

    Duties and Responsibilities of the Supervisor 

    To examine and provide feedback on supervisee’s case presentations, goals, techniques and intervention strategies. 
    To monitor and enhance skills based on the supervisee's level and experience. 
    To manage group dynamics, keep the content focused and support equality of time for each supervisee. 
    To uphold ethical guidelines and professional standards of ACA and ANZACATA. 

    Duties and Responsibilities of the Supervisee 

    To uphold ethical guidelines and professional standards. 
    To present client cases with the aid of written case summaries and/or recordings when required. 
    To be an active participant in group discussions. 
    To implement agreed upon interventions and considerations. 
    To consult supervisor or designated contact person in cases of emergency. 

    Privacy and Confidentiality 

    Session content will remain private and will not be discussed with others with the exception of the following circumstances: 

    If you are considered to be at risk of seriously harming yourself or someone else. 
    If your records have been requested by a court of law. 
    If another party or agency has requested your information, and you have agreed and provided your written consent to this. 
    Supervisor attends their own external supervision, and discusses issues related to the practice of supervision. 

    Note: Sessions are not to be recorded by any participant. 

    Record keeping 

    Clinical notes will be taken during or after each session. 

    These notes will be stored in a private and secure location and may be viewed by you if you so wish. Your records will be kept by the service for a period of seven (7) years from the date of your last contact with the service. 

    Cancellations

    Where supervision sessions are cancelled with less than 24 hours notice or a "no show" session will be charged at the full fee. The supervisee will be invoiced after each session and the invoice is payable the same day via EFT.

    Ending Supervision sessions 

    Speak to your supervisor if this group is not right for you. 

    This contract serves as a formal agreement between the supervisee and Melbourne Art Therapy Studio for the provision of supervision. By signing, I agree, to the best of my ability, to uphold the guidelines specified in this supervision agreement and to manage the supervisory relationship and supervisory process according to the ANZACATA and/or ACA (whichever membership is held) Standards of Professional Practice and Code of Ethics.
