Informed Consent and Acknowledgment
I, the undersigned parent or legal guardian, give permission for my child to participate in The Next Step: Growing Futures Summer Program, a four-week youth enrichment program hosted by Be The Change Empire in collaboration with Radiance Gardens and Otis Library.
I understand that the program is designed to provide youth with opportunities to develop life skills, leadership, communication, goal-setting, resilience, wellness, and community engagement skills through educational and hands-on activities. Program activities may include gardening, planting, nutrition education, group discussions, team-building exercises, journaling, creative activities, and other age-appropriate learning experiences.
I understand that some activities may involve hands-on gardening and the use of common gardening materials, soil, plants, seeds, and basic gardening tools. I understand that participation in these activities may involve minor risks, including but not limited to exposure to soil, plants, insects, outdoor elements, and minor cuts or scrapes. Reasonable precautions will be taken to promote a safe and supportive environment for all participants.
I understand that participation in the program is voluntary and that I am responsible for providing accurate and complete information regarding my child's health, allergies, dietary restrictions, medical needs, accommodations, and emergency contacts.
I understand that transportation to and from the program is not provided by Be The Change Empire, Radiance Gardens, or Otis Library. I am responsible for arranging transportation for my child to and from the program and for ensuring that my child arrives and departs safely.
I understand that my child is expected to follow program rules, respect staff, volunteers, partner organizations, and other participants, and participate in activities in an appropriate and respectful manner. I understand that program staff may contact me if concerns arise regarding my child's participation, safety, or behavior.
In the event of an emergency, I authorize program staff to contact the emergency contact(s) listed on this registration form. If emergency medical attention is necessary and I cannot be reached immediately, I authorize appropriate emergency personnel to provide necessary care for my child.
I understand that Be The Change Empire, Radiance Gardens, and Otis Library will take reasonable steps to provide a safe, welcoming, and supportive program environment. I acknowledge that no activity or program can be guaranteed to be completely free of risk.
By signing below, I confirm that I have read and understand the information provided in this Informed Consent and Acknowledgment. I voluntarily give permission for my child to participate in The Next Step: Growing Futures Summer Program and acknowledge my responsibilities as the child's parent or legal guardian.
