By signing this document, I acknowledge that I have been given the full opportunity to ask any questions about my tattoo procedure at Beneath The Surface Tattoo Studio, and all questions have been answered to my satisfaction. I confirm that I have read, understood, and initialled each section, and I voluntarily agree to the terms below.

• I confirm that I do not have diabetes, epilepsy, hepatitis, haemophilia, HIV/AIDS, tuberculosis, or any other communicable diseases, nor any heart conditions or blood-thinning disorders.

• I am not pregnant or nursing. I am not under the influence of alcohol or drugs.

• I do not have any skin or medical conditions (including but not limited to eczema, psoriasis, acne, keloids, moles, sunburn, or infections) that may interfere with the tattoo healing process.

• I understand that it is my responsibility to fully disclose any medical conditions that may affect the tattoo process or healing.

• I understand that allergic reactions to pigments, needles, equipment, or aftercare products can occur.

• I acknowledge that the artists and Beneath The Surface Tattoo Studio cannot guarantee whether I may experience an allergic reaction.

• I accept full responsibility for this risk and indemnify the studio and artists against claims arising from allergic reactions.

• Infection is possible with any tattoo procedure.

• I have been given aftercare instructions and agree to follow them exactly.

• Improper aftercare may lead to infection or complications.

• Any touch-up work required due to my negligence will be at my own cost.

• Movement during the tattoo session may affect the final result.

• Variations in skin tone or natural healing may affect colour and design.

• Tattoos on darker skin may appear less bright.

• The artist and studio are not responsible for the meaning or interpretation of symbols or foreign languages.

• A tattoo is a permanent modification of my body.

• No guarantees have been made regarding my ability to remove or alter the tattoo.

• I accept full responsibility for any emotional, mental, or physical effects of my decision.

• I confirm I am 18 years or older and have presented valid Photo ID.

• I understand the procedure and risks involved.

• I voluntarily release, discharge, and agree to indemnify Beneath The Surface Tattoo Studio and its
artists from all claims arising from:

- allergic reactions

- infections

- scarring or keloiding

- dissatisfaction with the final appearance

- complications caused by my skin type, health, or aftercare

• I consent to photographs or videos of my tattoo being used in promotional or digital media.

• I release all rights to these images without compensation.

• I am being tattooed by my own choice.

• I accept full responsibility for my decision.

• I indemnify the tattoo artist and Beneath The Surface Tattoo Studio against all liability.

• My tattoo may be performed off-site at events, activations, expos, conventions, festivals, or
temporary setups.

• Professional hygiene standards will be followed, but event environments are not controlled studio
environments.

• Factors such as lighting, foot traffic, and space may influence the tattoo process and healed
result.

• Design limitations may apply in event settings.

• I will not hold Beneath The Surface Tattoo Studio, its artists, staff, partners, or sponsors liable for
issues arising from the event environment.

• I assume all risks associated with tattoos performed in event settings.
