What This Form Does
We are documenting the neglect of disabled tenants, senior tenants, and children exposed to lead — including lead paint, chipping, and peeling paint — at Summit and Pinnacle. The information you provide will be used to build a clear record of conditions and to connect tenants with legal and government resources for enforcement.

This program exists especially for tenants who are older, do not use technology, or feel unable to advocate for themselves. If you know a senior or disabled neighbor in this situation, please help them fill out this form, or — with their permission — fill it out on their behalf using their information. No one should be left out because they can't use a computer or phone.

How Your Data Will Be Used
We will not voluntarily give this list to Summit, Pinnacle, or their attorneys. We will share it only with:

Attorneys we retain or refer this matter to.
NYC Housing Court (for filing official complaints).
NYC Department of Housing Preservation and Development (HPD) (for inspections and violation orders).
The NYC Mayor's Office for People with Disabilities (MOPD).
Your local City Council member's office (based on your borough and district).
NY State Division of Housing and Community Renewal (DHCR) — since this building is rent-stabilized, DHCR has jurisdiction over rent reductions caused by a reduction in building-wide services, such as loss of elevator service.
NYC Department of Health and Mental Hygiene (DOH) — DOH is required to monitor lead-related activity and compliance in the building.

Note: If this data is subpoenaed by a court or required by law, we may be compelled to disclose it. We will notify you if that happens.

⚠️ Anonymity note: Building and floor alone won't identify you if many tenants share a floor, but in smaller buildings it may narrow things down. We still won't share it outside this project, but if you want stronger protection, choose Tier 1 or 2 instead.

Tier 3 helps us prove a pattern of how the building owner and management handle these issues — it strengthens the overall case. But the further your information stays from Tier 1 or 2, the less specific information we have to pursue an individual resolution for your unit. If you want the best chance at getting your own situation resolved, Tier 1 or 2 gives us what we need to act on your behalf directly.

📌 Upgrade Process for Tier 2: If you choose Tier 2, and we file a complaint or retain an attorney, we may send you an email asking if you want to upgrade. To upgrade, you must reply with a clear statement such as: "I authorize Summit/Pinnacle Tenant Access Project to use my full name and unit number in legal filings from this date forward." A simple click will not be enough — we need your explicit written consent each time you move up a tier.

🛗 Reduction in Services — Elevator History (DHCR)

Buildings that are rent-stabilized, like this one, are required to maintain the services that were in place when tenants moved in. If a service like a working elevator is taken away or reduced, tenants may be entitled to a rent reduction through DHCR (the state agency that oversees rent-stabilized buildings). Many tenants don't know this option exists, so please answer the questions below as best you can, even if you're not sure.
