By signing below, I certify and agree to the following:Ownership & Authority: I am the legal owner (or authorized agent) of the pet described above and have full authority to surrender this animal.Permanent Transfer: I permanently transfer all legal rights, ownership, and custody of this pet to Twin Titans Inc. I understand that this surrender is final and irrevocable.Accuracy of Information: To the best of my knowledge, all information provided in this form is true, correct, and complete—including any known bite history or behavioral concerns.Veterinary Records Release: I authorize Twin Titans Inc to contact my veterinary clinic and request complete medical records for this pet.
Surrender Fees: All fees are non-refundable

A non-refundable fee is required upon the surrender of any animal to assist with intake, medical evaluation, and ongoing care.

Dogs (Over 30 lbs): $350 non-refundable fee per dog.

Dogs (30 lbs or less): $200 non-refundable fee per dog.

Puppies (under 1 years old) $200 non-refundable fee per puppy.

Kittens: $50 non-refundable fee per kitten.

Adult Cats: $50 non-refundable fee per cat. (Only spayed/neutered adult cats are accepted for surrender.)

Payment Terms: Payment must be made in full at the time of surrender via Cash or Zelle only. By signing below, the surrendering party acknowledges and agrees that all fees are strictly non-refundable under any circumstances, regardless of the subsequent placement, adoption, or status of the animal(s).
