• OLA Life Membership Nomination Form

  • Ontario Lacrosse Life Membership recognizes individuals whose exceptional dedication and service have made a significant and lasting contribution to the game of lacrosse and brought dignity to the Ontario Lacrosse Association.

    Submissions must demonstrate that the nominee meets the eligibility requirements outlined below and must include sufficient supporting information for the Life Membership Screening Committee to properly evaluate the nomination.

    Please review all requirements carefully before beginning. Incomplete nominations will not be considered.

  • Eligibility for Life Membership

    A nominee must:

    1. Have provided service to the game of lacrosse at the highest standard of dedication and in a manner that has brought dignity and respect to the Ontario Lacrosse Association.

    2. Have completed a minimum of 25 years of active service to the game of lacrosse as a builder, within or benefiting the Ontario Lacrosse Association and its membership.

    3. Have participated at a minimum of two of the following levels:
      1. Local
      2. Provincial
      3. National
      4. International

    4. Have contributed to the game in a non-playing capacity. For the purposes of Life Membership, a builder is an individual who contributes to the game in a non-playing capacity.

    5. Understand that any remuneration received for their service will be weighed against their volunteer contributions.

    6. May be either currently active or no longer active in lacrosse.
